  • The Boys & Girls Clubs of Monmouth County also used the following fields to learn more about your child. Please check one item from each group below.
  • I, the parent or legal guardian hereby give my permission for my child to participate in the Chill Zone/Sensory Room Programming at the Boys & Girls Clubs of Monmouth County. I fully understand that the program will involve staff members who are trained in Trauma-Informed Care and working closely with my child. Chill Zone activities include but are not limited to, sensory activities, social-emotional check-ins, pulse checks, learning coping mechanisms, and/or any other activities to help children become in tune with their emotions.
  • I have read the complete application, understand the rules of the Boys & Girls Clubs of Monmouth County and request that my son/daughter be admitted into membership. Although every effort will be made to prevent injury, if an injury should occur to my child while participating in Club activities, I agree to hold the Boys & Girls Clubs of Monmouth County, its agents and employees, and any volunteers/employees harmless.
    I understand by enrolling my child he/she will be part of an electronic student management system that complies with full confidentiality and Acceptable Use Policies of The Boys & Girls Clubs of Monmouth County.

    I grant the Boys & Girls Clubs of Monmouth County the unlimited right to use and/or reproduce photographs or likenesses in any legal manner for the internal or external promotional and information activities of the Boys & Girls Clubs of Monmouth County. I also agree to allow my child to be interviewed and/or photographed by representatives of the external news media in relation to any and all coverage of the Boys & Girls Clubs of Monmouth County in which he/she is involved. I also agree to allow my child’s work and/or photograph to be published on the Boys & Girls Clubs of Monmouth County Web site/Internet pages, and publications.     I give permission for my child to participate in BGCM Virtual Programming. Virtual programming will be an extension of our in-person programming, which is available for members at the Red Bank, Asbury Park, and 21st Century Community Learning Center locations of the Boys & Girls Clubs of Monmouth County. Virtual Programming will be available to our members as part of our BGCM Boundless virtual Club experience. BGCM Boundless (bgcmonmouth.org/boundless) was created for our Club members and their families so that they could participate in fun programs and activities while keeping in touch with their friends and Club leaders from home. Club members are able to engage in our programs and activities through our BGCM Boundless Facebook group, YouTube, Zoom meetings, and more. Our virtual program serves our members ages 5-12 in kindergarten through middle school and concentrate on academic support, performing arts, and other related topics.

    PLEASE NOTE THAT ANNUAL CLUB MEMBERSHIP FEES ARE NOT TRANSFERABLE BETWEEN UNITS NOR REFUNDABLE.

  • Boys & Girls Clubs of Monmouth County's sites at College Achieve, Martin Luther King Jr. Middle School, and Bradley Elementary for grades 3-8 receive 21st Century funding, and the following statement applies to that project:

    “This project was funded in its entirety Under Title IV, Part B of the Every Student Succeeds Act (ESSA), 21st Century Community Learning Center (21st CCLC) grant funds through a grant agreement with the New Jersey Department of Education.”
